By Mail:

Please download the donation form (PDF), and mail it with your check to the Division of Hematology, Attn: Elaine Staats, 269 Campus Drive, CCSR 1155, Stanford University School of Medicine, Stanford, CA 94305-5156

Please include your name, address, phone number, and e-mail address.

Online:

Visit the Stanford University Giving Site.  **Be sure to reference the "Schrier Fellow Scholar Award (HAHRL)" in the "Special Instructions" window of the form.  This will ensure that your gift will be placed into the correct fund.  Below are specific step-by-step instructions that will walk you through the process of making an online donation.

Step-by-step Instructions to Make an Online Donation to the Schrier Fellow Scholar Award

1. Click on this link: Giving Site
2. Click Make a Gift (bottom right-hand side of page)
3. Select "Stanford Medical Center"
4. Select "One-Time Gift" for a single payment or "Recurring Gift" if you plan on making recurring payment (for example a $200 donation every year for 5 years)
5. Select the method of payment you would like to use (options are credit card, wire transfer or security transfer)
6. Click "Continue"
7. Under the "Further Designation" heading select "School of Medicine-Other (specify below)"
8. Indicate the amount you want to donate in the box
9. Type "Schrier Fellow Scholar Award (HAHRL)" in the Special Instructions box to direct your donation specifically to this fund.
10. Complete the personal information boxes and continue to the next page to submit gift online
